After completing the online registration, all required paperwork detailed below must be submitted to to Karen Jemilo via email at kjemilo@district146.org. Those wishing to schedule an in-person appointment should contact Karen Jemilo to do so. Walk-ins will not be accepted. All registration steps must be completed, and residency and custody must be established and approved before a child can be enrolled.
In order to register, the following are required:
- The child's original birth certificate
- Both parents’ current driver’s license or other government-issued photo identification card
- Establish residency within District 146 by providing the following documents:
Category I (One Document)
Category II (Three Documents)
- Home ownership title
- Most recent property tax bill and proof of payment
- Monthly mortgage statement (less than 30 days old)
- Apartment lease (complete, fully executed) or affidavit from landlord in lieu of lease (either must be signed and dated, with lessee(s) and occupant(s) of the unit listed), and proof of last month’s payment
- Current Illinois driver’s license or other government-issued photo identification card
- Current Illinois voter registration
- Current vehicle registration
- Current homeowner’s or renter’s insurance
- Auto insurance
- Cable bill
- Internet services
- Most recent gas, electric, and/or water bill (within last 3 months)
- Current public aid card
- Paycheck stub (less than 30 days old)
- Month-to-month, open-ended, or altered leases will not be accepted without a signed statement from the landlord verifying the validity of the lease.
- P.O. Box addresses are insufficient as proof of residency.
- Residency Verification through CLEAR. (District 146 uses CLEAR public database to verify family address information. CLEAR uses parent/guardian’s name and address to crosscheck provided information against a public electronic database.
If the parent/guardian is not the District 146 resident and the child and parent/guardian are residing with someone else in the District 146 below are the requirements:
- The District 146 resident must submit all the required Category I and Category II documents referenced above.
- The District 146 resident must provide current government-issued photo identification.
- The District 146 resident must complete the student residency affidavit provided by District 146. Please note that residency affidavits along with all needed documentation must be renewed and submitted annually for each school year.
- The parent/guardian must also submit the required Category II documents.
Please note:- All documents must be deemed acceptable by District 146.
- Only documents that include parent/guardians' and District residents’ legal name and current address will be accepted as proof of residency.
- District 146 may require a home visit and/or additional documentation to verify residency.
- District 146 actively investigates all residency.

Custody typically refers to the day-to-day care and physical residence of a child, which must be established and approved by the District before the child can be enrolled in school. Guardianship involves the legal authority to make major decisions for the child and requires specific legal paperwork to be submitted if the person registering the child is not the biological parent.
- Custody (Parental Responsibility): This typically refers to the rights and responsibilities of biological or adoptive parents. For registration, the district requires custody to be established—often through a birth certificate or a divorce decree/court order specifying which parent has the primary "regular fixed night-time abode" for the child.
- Guardianship: This is a legal relationship granted to someone other than a biological parent (such as a grandparent or relative). A guardian has the legal authority to make "day-to-day" and "major" decisions for the child, including educational placement. For school purposes, a guardian must provide court-appointed guardianship paperwork or a valid short-term guardianship form to prove they are the child's legal representative.
Registration Requirements
- Residency: Both custodians and guardians must prove residency within District 146 boundaries using required documents like a home ownership title, lease, or utility bills.
- Approval: Registration is not considered complete until the District Office reviews and approves both residency and the appropriate custody or guardianship paperwork.
- Temporary Situations: For certain temporary arrangements, the district may require updated documentation or reviews as frequently as every six months.
- State Law: In Illinois, state law and the federal Individuals with Disabilities Education Act (IDEA) require that parents or legal guardians be the ones to enroll a child and initiate or consent to the Individualized Education Program (IEP) process.

In Illinois, student health requirements are strictly regulated to ensure a safe and healthy learning environment. Based on the registration requirements for School District 146, there are three primary medical forms mandated by the state that must be completed before the start of the school year.

May 15 General Health Requirements for All Transfer Students
- Physical Examination: All transfer students must present proof of a current physical exam.
- Deadline: Required health forms must be presented to the District by October 15. Students who enter mid-year have 30 days from their first day of attendance to comply.
- Exclusion Policy: Failure to comply by the October 15 deadline will result in the student's exclusion from school until the required forms are submitted.
- Immunizations: New students must show proof that all vaccinations are current. Required vaccines include DTP, polio, MMR, Varicella, Hepatitis B, and HIB.
- Dental Examination: Required for students in kindergarten, 2nd, and 6th grades. The completed form must be returned by May 15.
- Eye Examination: Required for all students (grades 1-8) entering the Illinois school system for the first time. Proof of examination by a licensed ophthalmologist or optometrist must be returned by October 15.
Submission of Medical Information
All medical information, including health exemptions and medical action plans, must be submitted directly to the student's school nurse. These documents should not be sent to the district office.
Additional Health Factors
- Emergency Action Plans: Students with asthma, significant allergies, diabetes, or seizure disorders must submit a completed Emergency Action Plan and/or Diabetes Medical Management Plan signed by a healthcare provider. These plans are valid for one year.
- Sports Physicals: Any student trying out for a team sport at Central Middle School must have an updated physical turned in to the nurse prior to tryouts. These are valid for 395 days from the date of completion.
- Physical Examination: All transfer students must present proof of a current physical exam.

If your child has an identified disability requiring a Section 504 Support Plan or specialized instruction through an Individualized Education Program (IEP), please submit the most recent plan to the District Office as soon as possible. This plan will be forwarded to your child's school of enrollment for review by the Student Services Team prior to a start date being issued. This step is necessary to determine an initial plan of support for your child.

District 146 uses a convenient service for paying for school fees and funding the food service account called PushCoin. Here are instructions on setting up a PushCoin account are available.
Following registration, families will be notified via email regarding registration fees. All Registration Fees are nonrefundable and are not prorated.
Parents or guardians will be subject to the payment of retroactive tuition charges for nonresident students.

Fees for the 2026-2027 school year will be applied after April 1, 2026. -
The Birth to Five Program is comprised of two programs: The Bridges (Birth to Three) Program serves families with children from birth to age three as well as expectant parents. Our Early Learning Prekindergarten program provides half-day preschool programming for eligible students between the ages of 3-5.
Both programs are funded by grants from the Illinois State Board of Education and serve children and families with socio-economic, developmental, and environmental risk factors. These programs provide educational opportunities, connections to community resources, as well as parenting support throughout the early childhood years for qualifying families residing within district boundaries.
